An administrative process is rarely broken in the middle; it breaks at the handover, where responsibility is ambiguous and nobody owns the queue. The method here is observational first: the process is documented as it is genuinely performed, not as the manual describes it. Redesign follows, and it is deliberately conservative — the smallest change that removes the stall, written down as a routine somebody can follow without the adviser in the room.
